Adult Recreational Soccer Liability Waiver and Release
By registering for and participating in any soccer event organized by Lift Up Media LLC (DBA Urban Fútbol Club) ("Organizer"), I acknowledge and agree to the following:
I understand that participation in recreational soccer involves inherent risks, including but not limited to serious bodily injury, permanent disability, paralysis, and death. I voluntarily assume all risks associated with participation, whether known or unknown, even if arising from the negligence of others.
I hereby release, waive, discharge, and hold harmless Lift Up Media LLC, its owners, officers, agents, hosts, representatives, and affiliates (collectively, the "Released Parties") from any and all claims, liabilities, damages, losses, or expenses arising out of or related to my participation in any event.
I acknowledge that the Organizer does not own, control, or maintain the facilities where events take place. I understand that field conditions may be uneven, unsafe, or hazardous. I further acknowledge that injuries may result from actions of other participants, spectators, or non-participants, and I assume full responsibility for such risks.
I understand that the Organizer does not provide medical insurance or coverage. I am solely responsible for any medical expenses incurred as a result of participation.
I agree to play responsibly, follow general safety guidelines, and refrain from reckless or dangerous behavior. I understand that the Organizer may remove participants for unsafe conduct.
I certify that I am physically fit and have no medical condition that would prevent safe participation in soccer activities.
I agree to indemnify and hold harmless the Released Parties from any claims brought by third parties arising from my actions during participation.
I understand that this waiver is intended to be as broad and inclusive as permitted by the laws of the State of Florida. If any portion is held invalid, the remaining provisions shall continue in full force.
I understand that photographs, video recordings, and other media may be taken during events organized by Lift Up Media LLC (DBA Urban Fútbol Club). By participating, I grant the Organizer and its affiliates the unrestricted, irrevocable right and permission to capture, use, reproduce, distribute, and display my name, image, likeness, voice, and appearance in any media now known or hereafter developed, including but not limited to social media, marketing materials, advertisements, and promotional content, without compensation or further approval. I waive any right to inspect or approve the final use of such materials and release the Organizer from any claims arising out of or related to the use of such media, including any claims for compensation, invasion of privacy, or misappropriation of likeness.
By registering and/or participating, I confirm that I have read, understood, and voluntarily agree to this waiver and release of liability.
